The Trial Measured the Disease and Found Nothing

D-CARE ran for four years and enrolled 2,176 families. It tested what almost nobody tests, whether organizing dementia care around a navigator actually changes anything. The answer came back split. Cognition did not move. Daily functioning did not move. Quality of life for the person with dementia did not move. Caregiver satisfaction moved, and it moved by three months and stayed moved through eighteen. Community-based programs edged out health-system ones.

That split reads as a disappointment and it is not. The trial measured the disease and found nothing, then measured the family and found something. Most caregiving products are sold on the first claim and delivered on the second. CMS built the GUIDE model on this premise, and the evidence for it has been sitting in plain view for a year.

A family can take one thing from it. Better coordination will not slow the illness. It will change what the week feels like, which is not nothing.
