The number worth sitting with in Trilliant Health's analysis isn't the 37.5x. It's the pairing. Biomarker testing grew thirty-seven-fold between 2023 and 2025, while geriatric medicine fellowship fill rates fell from 58.7% to 38.9%, against a specialist shortfall that already covered 79% of hospital referral regions in 2020. Detection got fast and cheap. The people who explain what detection means did not.

Mild-stage cases are now 48.7% of new diagnoses. That is a different family walking in. They are not in crisis at 2 a.m. after a fall. They have a result, a ten-year runway, and a referral queue with a two-year wait at the end of it. Most of this category was built for the crisis family, because the crisis family is the one who calls. Nobody built much of anything for the family that got told early and then got sent home to wait.
